How to Use Our Customer Satisfaction Survey Template

1. Get this survey template in Excel or PDF, or make your own copy in Google Sheets.

2. Complete the columns by recording all customer feedback details:

  • Date: Enter the date when the customer feedback was collected or the survey was completed.
  • Customer Name: Record the customer’s full name for feedback reference and follow-up.
  • Email: Include the customer’s email address to enable direct communication and response tracking.
  • Product/Service: Specify the particular product purchased or service received that is being evaluated.
  • Quality (1-5): Rate the customer’s assessment of product/service quality on a scale from 1 (very poor) to 5 (excellent).
  • Support (1-5): Document the customer’s rating of customer service and support experience from 1 (very poor) to 5 (excellent).
  • Delivery (1-5): Record the customer’s satisfaction with delivery timeliness and condition on a scale from 1 (very poor) to 5 (excellent).
  • Overall Satisfaction (1-5): Capture the customer’s overall satisfaction rating on a scale of 1 (very dissatisfied) to 5 (very satisfied).
